Uber Eats charges restaurants in Sri Lanka 30% of the order when its couriers deliver and 15% on pickup, plus a one-time LKR 1,000 activation fee. Card processing is included, and VAT is added to the fee.

Included in the commission. Uber Eats lists payment processing in every plan, with no separate card fee.
VAT on the commission
Yes. Uber's Sri Lankan terms say its fees exclude indirect taxes such as VAT and the Social Security Contribution Levy (SSCL), which the restaurant pays on top where they apply. VAT is 18% in Sri Lanka; a VAT-registered restaurant can usually claim it back.
Activation fee
LKR 1,000, once, taken from your payout after your first LKR 1,000 of sales. It covers menu creation and a welcome pack.
